Take the Stroke Risk Assessment Test

Take this test to find out if you are at risk for a stroke.

1. Do you smoke? Yes No Quit
2. Are you diabetic? Yes No
3. Do you take insulin? Yes No
4. Do you have high cholesterol? Yes No Don't know
5. Do you take medicine for high cholesterol? Yes No
6. Do you have high blood pressure? Yes No
7. Do you take medicine for high blood pressure? Yes No
If you are over 50 years old and answered "yes" to two or more of the above questions, you may be at a higher risk for stroke.


8. Have you ever had a heart attack, heart bypass, or angioplasty? Yes No
9. Have you ever had blockages in the circulation to your legs? Yes No
10. Have you had bypass surgery or angioplasty? Yes No
11. Has anyone in your immediately family had a stroke? Yes No
12. Has a doctor ever told you that you have blockages in your neck arteries? Yes No
If you answered "yes" to any of the questions above, you may be at higher risk for stroke. Early detection and treatment in most cases can significantly reduce or even eliminate the chance of suffering disabling paralysis or death from a stroke.


13. Have you ever had a stroke, or a "mini-stroke" (TIA)? Yes No
14. Have you ever had weakness, numbness or paralysis of your arms or legs? Yes No
15. Have you ever had sudden blurred vision or blindness in your eye? Yes No
16. Have you ever had difficulty speaking or slurring of your speech? Yes No
17. Have you ever had severe headaches that occur for no reason? Yes No
18. Have you ever lost your balance or fell down for no reason? Yes No
If you answered "yes" to any of the questions above, you may be at extremely high risk for a future stroke.
