To get a copy of your medical records from your hospital stay, lab tests or diagnostic imaging, you must fill out and sign a release form, see links below. Your medical records are confidential documents, protected by privacy laws.

 

Please print and completethe appropriate form below, or call us and we will fax the form to you. You mustreturn your release form to the Medical Center (address below)in person and provide photo identification,such as adriver's license or identification card.

There is no charge for medical records sent to your health care provider for your continuing health care. If you are requesting records for your personal files, the charge is$1 per page. Total cost depends on the number of pages in your record.
